Introduction to Cocaine Addiction​

Cocaine’s history dates back to the 15th century, where it was initially utilized for various medical purposes, most notably as a surgical anesthetic. While it remains legal for specific medical applications, its use is now rare. This is due to the extensive knowledge we have gained about its high potential for misuse and the severe risk of developing a cocaine addiction.

Cocaine is a powerful stimulant drug derived from the coca plant, which is native to South America. On the illegal market, cocaine is often “cut” or mixed with other white, powdery substances like cornstarch, talcum powder, or even flour. This is done to increase the volume of the product and maximize profits for dealers. A far more dangerous practice is mixing cocaine with potent synthetic opioids like fentanyl. This creates a deadly combination, especially for unsuspecting users who are unaware of the fentanyl’s presence, dramatically increasing the risk of fatal overdose and complicating the path away from cocaine addiction.

Similar to other substances of abuse, cocaine directly impacts the brain’s neurochemistry by increasing the levels of dopamine, a natural neurotransmitter. Dopamine is a key component of the brain’s reward system, responsible for feelings of pleasure and euphoria. The intense “high” produced by this dopamine surge reinforces drug-taking behavior, driving individuals to use the drug repeatedly to chase that initial euphoric feeling. This cycle is the very mechanism that makes the drug so highly addictive and is the foundation of cocaine addiction. Many individuals who abuse cocaine find that they can never replicate the intense pleasure of their first use, yet they continue to try, a hallmark of a developing cocaine addiction.

With long-term cocaine abuse, the brain begins to adapt to the consistent presence of the drug. This adaptation leads to a state known as tolerance, where the brain becomes less sensitive to cocaine’s effects. As a result, the individual must consume progressively larger and more frequent doses to achieve the same desired high. This escalating use pattern is a clear sign of a worsening cocaine addiction and significantly increases the risk of overdose and other severe health consequences.

The prevalence of cocaine use and cocaine addiction is a significant public health concern. According to the National Institute on Drug Abuse, a 2020 study revealed that an estimated 1.9% of individuals in the U.S. over the age of 12 reported using cocaine within the past year. Within that group, the same study found that approximately 0.5% met the criteria for a diagnosable cocaine use disorder, highlighting the transition from casual use to a full-blown cocaine addiction for many.

A 2021 study indicated that preteens and teenagers are experimenting with the drug, with 1.2% of high school seniors and 0.2% of 8th graders reporting cocaine use in the past year.  At a local level, the impact of cocaine addiction is also evident. In Kentucky, for instance, state data from 2020 showed that 1,178 individuals sought and received treatment specifically for smoking cocaine, demonstrating the critical need for accessible and effective treatment programs to combat cocaine addiction.

Signs & Symptoms Of Cocaine Addiction

Cocaine can be misused by individuals who do not yet have a cocaine addiction, and it is commonly abused in social situations. However, this casual use can quickly spiral into a full-blown cocaine addiction. Recognizing the signs that someone is under the influence of cocaine is the first step toward helping them address a potential cocaine addiction. These signs and symptoms can include some or all of the following:

  • Decreased appetite: Cocaine is a powerful stimulant that suppresses appetite. A person struggling with cocaine addiction may experience significant, unexplained weight loss over time.

  • Irritability and agitation: As the drug’s effects wear off, users can become extremely irritable, anxious, and agitated. This emotional volatility is a key behavioral sign linked to cocaine addiction.
  • Paranoia: Cocaine can induce intense feelings of paranoia and suspicion. An individual may feel like they are being watched or are in danger, a psychological symptom that often worsens with a deepening cocaine addiction.
  • Muscle twitches or tremors: The stimulant effects can cause involuntary muscle movements, twitches, or tremors, especially in the face and fingers.

Cocaine Withdrawal

When an individual grappling with cocaine addiction abruptly stops or significantly reduces their cocaine use, they will almost certainly experience withdrawal symptoms. The severity and duration of these symptoms can vary depending on the extent of their cocaine addiction. Symptoms that commonly manifest during cocaine withdrawal can include some or all of the following:

  • Intense Cravings: An overwhelming psychological urge to use cocaine again is a hallmark of withdrawal and a major challenge in overcoming cocaine addiction.
  • Fatigue and Exhaustion: A profound sense of tiredness and lack of energy often follows the cessation of cocaine use.
  • Irritability and Agitation: Individuals may feel easily annoyed, short-tempered, and restless.
  • Anhedonia (Lack of Pleasure): A diminished ability to feel pleasure from everyday activities is a common and distressing symptom.
  • Vivid and Unpleasant Dreams: Many report experiencing intense and often disturbing dreams.
  • Paranoia and Suspicion: Feelings of distrust and paranoia can emerge or intensify during withdrawal.
  • Depressed Mood: A significant “crash” often occurs, leading to symptoms of depression.

It’s important to understand that while some drug withdrawals involve severe physical symptoms like vomiting, cocaine withdrawal is primarily psychological. For this reason, it is not typically considered a life-threatening health crisis in the same way as alcohol or opioid withdrawal. However, this does not diminish its difficulty. For some, the psychological distress from cocaine withdrawal can be extreme, leading to severe depression and suicidal thoughts. In these cases, medical treatment within a detox facility is highly recommended to manage the symptoms and ensure the person’s safety, providing a crucial first step in treating their cocaine addiction.

 A significant risk to consider is that street cocaine is often “cut” or mixed with other substances, such as opioids like fentanyl. If a person’s cocaine addiction involves a substance that is contaminated with opioids, their withdrawal experience will be dangerously different and likely include severe physical symptoms. Opioid withdrawal symptoms such as persistent vomiting and sweating can lead to severe dehydration, electrolyte imbalances, and other complications, quickly becoming a medical emergency that requires immediate professional attention. This hidden danger underscores the importance of seeking medically supervised detox for any level of cocaine addiction.

Cocaine Behavioral And Physical Changes

This method of use also increases a person’s risk for dangerous infections such as pneumonia, which are common issues for those with a severe cocaine addiction.

Intravenous cocaine users face a heightened risk of contracting bloodborne diseases like HIV, Hepatitis B, and Hepatitis C, especially if they are sharing or using unsterile needles and equipment. This practice can also lead to collapsed veins, skin infections, and permanent scarring at the injection sites, all direct physical results of cocaine addiction.

The behavioral changes that occur with cocaine addiction can look similar to those of other addictions. It’s common for individuals to encounter significant problems at work or school, often leading to job loss or academic failure. A cocaine addiction frequently causes strained and challenging relationships with loved ones and friends as the drug becomes the central focus of the person’s life. They may also encounter legal troubles due to possession or engaging in more impulsive and risk-taking behaviors to fund their habit.

You may notice that an individual with a cocaine addiction has stopped engaging in activities and hobbies they used to enjoy. They might not follow through on commitments or responsibilities because they are preoccupied with obtaining and using the drug. Another sign of cocaine addiction is finding paraphernalia such as straws, rolled-up bills, small baggies, or a pipe in their possession.

Recognizing the symptoms of a cocaine overdose is critical for a rapid response. These symptoms can include some or all of the following:

  • A dangerously high body temperature (hyperthermia)
  • Rapid or irregular heart rate

  • Nausea and vomiting

While a person may not think they took an opioid, it’s impossible to know for sure what’s in a street drug without a lab test. Narcan is a medication designed to rapidly reverse an opioid overdose and can be a life-saving measure if an individual has unknowingly consumed fentanyl. It is important to understand that Narcan will not reverse an overdose caused solely by cocaine; however, it can save a life if opioids are also present and will not cause harm if they aren’t. Treating cocaine addiction often involves addressing the polysubstance use that makes overdoses so deadly.
